Step One: Get Fingerprinted

Have fingerprints taken at either one of our partner locations, where they can submit the fingerprints and forms electronically, or you can get fingerprinted at most local law enforcement agencies on ink cards for a nominal fee. When being fingerprinted, make sure it is on the FBI standard FD-258 cards.

The FD-258 Form is also available online at FBI.gov

If sending in ink cards, please send two complete sets of fingerprints. In some cases, the FBI might reject a set of fingerprints due to quality. If this happens, the second set will be submitted immediately, reducing your wait time for a completed report. If the first set of fingerprints is rejected, PrintScan will submit your second set FREE of charge. If your second set of fingerprints are rejected, the third submission will begin a new transaction. Please note, rejections are considered an FBI result and you will not be eligible for a refund if your fingerprints are rejected.

Step Two: Complete Forms

Complete an FBI release form, a credit card authorization form, and if necessary, an Attorney Release letter. If the results must be sent to your attorney, you will need to include an Attorney Release letter when mailing in your paperwork. (*See details below.)

NOTE: The FBI has certain rules and regulations when requesting a criminal history report. Please review the items below very carefully to ensure there are no delays once your paperwork is received.

  • An FBI-approved Channeler may only process requests for a U.S. Citizen or a lawful permanent resident of the U.S.). A lawful permanent resident is any person not a citizen of the U.S. who is residing in the U.S. under legally recognized and lawfully recorded permanent residence as an immigrant (also known “Permanent Resident Alien,” “Resident Alien Permit Holder,” and “Green Card Holder”).
  • An FBI-approved Channeler cannot process a request for employment and/or licensing purposes within the United States. This type of request should be coordinated with the appropriate State Identification Bureau (or state police) for the correct procedures.
  • Addresses provided on the release form must be residential addresses within the United States and reports may not be sent to the attention of anyone other than the individual who was fingerprinted. Business addresses and PO Boxes will not be accepted by the FBI.
  • Email addresses provided must be unique personal email addresses and not indicate the name of anyone other than the individual who was fingerprinted.

Step Four: Submit Order

If you come into one of our partner locations, they will submit your paperwork and fingerprints on your behalf, making the process simple and quick!
When mailing in your request please send all required information to our address below. Please note if something is forgotten it will slow down the process. We recommend sending your fingerprints with some sort of tracking, so you can make sure it is delivered. Please review the mailing checklist prior to the shipment of your paperwork.

Payment Types:

For payments with Visa, Mastercard, American Express and Discover, please include a completed and signed credit card authorization form. When paying with money order, please make the payment payable to PrintScan.
